At the upper left corner, a photograph of Eunice Jackson in an oval shape is superimposed on top of the photograph of Jack's Memory Chapel. At the upper right corner, a photograph of Samuel M. Jackson in an oval shape also is superimposed on top of the photograph of Jack's Memory Chapel. There is tape all along both sides of the top border of the photocopy.
