A freight train derailed at Medfield Junction, Medfield Mass., on October 22, 1923. The accident was caused by the failure of an engineman to properly observe and obey signal indication. The accident resulted in the death of two employees and the injury of four employees (Summary of Accident Investigation Reports, No. 1, Interstate Commerce Commission Bureau of Safety, 1919).
