This printed form was issued to the postmaster at East Kingston, New Hampshire for his failure to cancel a postage stamp placed on a letter mailed from his post office. The postmaster at Concord, New Hampshire, had reported him to the Washington headquarters. Signed by Alex. W. Randall, First Assistant Postmaster General (Appointment Office), the letter is addressed Washington, D.C., December 1, 1863. The form is printed on the front of a single sheet with blank areas for handwritten information. The document reads:
