The postmaster at Concord, New Hampshire, had reported him to the Washington headquarters. Signed by Alex. W. Randall, First Assistant Postmaster General (Appointment Office), the letter is addressed Washington, D.C., December 1, 1863. The form is printed on the front of a single sheet with blank areas for handwritten information. The document reads:
